Renewing your passport and/or amending your legal name on your passport

Submit Form DS-82 if ALL of the following are true:

  • You will submit your most recent passport;
  • You were at least 16 years old when thea t passport was issued;
  • That passport was issued less than 15 years ago.

Applications to renew a damaged passport via mail can be submitted if all identifying information is still legible. If the photograph or identifying information is illegible, refer to the information regarding Lost/Stolen/Damaged passports.

Completing the application

Please type or print legibly when completing all sections of the application. Please print out and submit only one-sided pages of the application.

Please disregard instructions and addresses on the online forms. Do not submit your application directly to the United States - it will not be accepted, and will delay processing of your application. Please submit the following by mail to the Consulate General serving your area:

  • Fully completed passport application (Form DS-82);
  • Most recent US passport;
  • Two (2) U.S. sized passport photographs. (Note: these are a different size from Australian passport photographs. See our list of known photographers);
  • Self-addressed Express Post Platinum or Registered Post envelope for return of your new and old passports;
  • Applicable Fee* (All fees are subject to change without notice);
  • If applicable — documentation to support name change or change of other descriptive data;
  • If applicable — a signed statement outlining the reason the passport is damaged.

* If you are applying for a new passport to correct an error in your identifying data (e.g. spelling of name, date or place of birth) due to a printing error by Passport Services, there is no fee. You will be issued a new passport with a new number and a new issue date, but with the same expiration date of the replaced passport.

Photo Guidelines

Two identical color photographs required 50mm × 50mm (2" × 2"); headsize chin to top of head minimum 25mm (1"), maximum 35mm (1 3/8").
